SAM Support Coordination services are committed to supporting individuals and their families throughout the lifespan. We recognize that individual needs and their families can change overtime and throughout the lifespan, therefore, we take an individualized and person-centered approach to our efforts and support.

We recognize and value that individuals, their families, and communities are rich in resources. We believe in teamwork and serving others. Therefore, we ensure that our staff have the necessary resources, information, and knowledge to help individuals, their families navigate their communities and the developmental disability service system so that persons served can define and achieve their vison of a good life.

SAM provides the following Supports Coordination Services to individuals with developmental disabilities and their families to help individuals achieve their vision of a good life:

  • Development of trusting relationships that are grounded in mutual respect and dignity

  • Creation and maintenance of an individualized support plan that identifies meaningful and measurable outcomes

  • Assessing the need for services and funding

  • Accessing supports in the community

  • Locating service providers and agencies

  • Coordination of services and team members

  • Monitoring to ensure that individuals served are receiving services and supports as outlined in their individualized plan

  • Monitoring to ensure the health, safety, and well-being of individuals served
